This Role Specific Addendum for Product Reviewer is an addendum to that certain Master Network Member Agreement between the parties.
WHEREAS, the Company desires to engage Product Reviewer to perform the services described in the Network Member Agreement and herein;
NOW, WHEREFORE, in consideration of the promises, covenants and obligations set forth in the Master Network Member Agreement and below, the sufficiency of which is hereby acknowledged, the parties hereto agree as follows:
Services. During the Term of the Master Network Member Agreement and while performing assignments specific to the role of Product Reviewer , the Product Reviewer agrees to:
a) Complete the requisite Product Reviewer training program prior to providing any review Services, and complete all subsequent training updates;
b) Compile results of reviews on the designated online form:
1. Record the brand and generic names of each product discussed or mentioned in the content
2. Identify the Manufacturer for each product recorded
3. Indicate whether the therapeutic option discussed is a primary theme of the discussion or simply mentioned in the content.
4. Determine the FDA-approval status for each use of the recorded product - e.g., although product may be approved by the FDA it may not be approved for the particular use discussed
5. For unapproved uses:
i. Identify the unapproved indication discussed (specifically; population, dosage, regimen, administration).
ii. Indicate location in the content of the off-label/investigational use discussed
c) Perform research as necessary to complete the product compilation form;
d) Respond to questions or requests for clarification from the Company about the products identified if needed;
e) Complete Company’s process and submit to the Company within two (2) hours of receipt of content or agreed upon deadline to ensure that Company can deliver the report to client by the agreed due date and time.
f) If unable to complete the confirmed assignment by the agreed upon deadline, notify Company’s Project Manager immediately.
g) Upon completion of the project, complete the online feedback form.
THE PRODUCT REVIEWER ACKNOWLEDGES THAT HE OR SHE HAS NOT BEEN PROMISED ANY LEVEL OR AMOUNT OF WORK OR FEES HERERUNDER.
This Addendum supplements but does not supersede the Master Network Member Agreement between the parties.
